Divine Health & Wellness is a medical wellness clinic located in downtown Sapulpa, Oklahoma. Divine exists to help people look and feel their best throughout their continuum of life. Our knowledgeable health professionals work with you to provide a treatment plan to help you reach your personal and wellness goals.

Matthew J Lee

Owner, APRN-FNP

Matt graduated from the University of Oklahoma with his Masters as a Nurse Practitioner in May of 2003. Over his 20 plus years of practice he has specialized in Family Practice, Urgent Care and has had experience in Cardiology. He is originally from Beggs, Oklahoma and currently resides there with his wife and kids.

Carlyn Tackett

APRN-FNP

Carlyn has over 19 years’ experience in the healthcare field. She first started her healthcare journey as a Certified Surgical Technologist, which fueled her desire to care for people even more.

Kristina Stout

APRN-FNP

Kristina Stout is a certified Family Nurse Practitioner with 10 years of experience working in the healthcare industry. She received her Bachelor of Science in Nursing from the University of Oklahoma and her Master’s from Texas Woman’s University.

Jessica Mooney

Clinic Manager

Jessica has over 7 years of experience in the healthcare field. Her journey began at St. Francis Children’s Hospital in Tulsa. From there during the pandemic, Jessica took her commitment to healthcare further by traveling across the country to assist in the fight against COVID-19.

Samantha Bryan

Director of Operations

Samantha is a highly skilled Director of Operations with over 10 years of experience in accounting and business. Graduating from Southern Nazarene University (SNU) with a bachelor’s degree in Business

Zoe Benton

Medical Assistant

Our Medical Assistant, Zoe, has lived in Oklahoma her entire life and previously worked in a medical office prior to joining our team. Zoe eagerly anticipates the opportunity to contribute to our team and ensure your experience at Divine is nothing short of exceptional.

Heather Taber

Client Service Specialist

Heather is the new Client Service Specialist at our office and brings a warm, welcoming presence to the team. A Sapulpa local, Heather is known for her excellent people skills and is always ready to assist with a smile.
